AbstractIntroduction: Cardiac hydatid cyst is an exceptionally rare condition in both developed and developing countries. Here, we present a case of a massive, asymptomatic myocardial hydatid cyst in a middle-aged man. Case Presentation: A 51-year-old man presented to the Emergency Department (ED) with complaints of exertional dyspnea and recurrent palpitations. He reported experiencing atypical chest pain. Echocardiography revealed a large mass in the posterolateral portion of the left ventricular wall and a giant mass in the basal portion of the posterior left ventricular wall. A CT scan confirmed the presence of massive cysts, measuring approximately 7.50 × 5.19 cm. The patient underwent cardiac surgery for the excision of multiple cysts of varying sizes, with no involvement of other organs. Conclusions: Early diagnosis followed by precise surgical intervention is essential for cardiac hydatid cysts, even in asymptomatic cases. This approach can help prevent potential life-threatening complications in patients.en
