AbstractBackground: Chronic kidney disease (CKD) is a significant public health challenge with high morbidity and mortality. Objectives: The present study aimed to investigate epidemiological characteristics, identify associated factors, and evaluate dialysis adequacy in CKD patients undergoing hemodialysis at two dialysis centers in Birjand and Tabas, Iran. Methods: This cross-sectional study was conducted on 184 hemodialysis patients using a census sampling method in South Khorasan province from February to December 2023. Data were collected from medical records and a validated questionnaire, measuring clinical (e.g., blood pressure, weight) and laboratory parameters (e.g., urea, creatinine). Dialysis adequacy was assessed via Kt/V and urea reduction ratio (URR). Descriptive statistics (mean ± SD), t-tests, chi-square tests, and correlation analyses were used to analyze quantitative variables, considering confounders like age and sex. Standardized data collection methods were used to minimize bias. Results: The majority of hemodialysis patients were older, male, literate, and unemployed. Furthermore, participants (70.7% male) showed that most achieved adequate dialysis (Kt/V ≥ 1.2; URR ≥ 65%). Kt/V and URR correlated with age, weight, and blood pressure, with higher adequacy in single and female patients. Conclusions: Despite sociodemographic influences on CKD, dialysis quality was satisfactory. These findings underscore the importance of integrating socioeconomic factors into patient management strategies to further optimize dialysis outcomes, particularly for vulnerable populations. Targeted interventions and longitudinal studies are needed to address these factors and confirm findings.en
