Comparative Effects of Dynamic Neuromuscular Stabilization Alone and Combined with Hip Strengthening on Balance, Strength, and Disability in Older Women with Chronic Low Back Pain

AbstractBackground: Chronic low back pain in older women often leads to reduced balance and strength, as well as increased disability, negatively affecting quality of life. Objectives: This study investigated the effects of dynamic neuromuscular stabilization (DNS) alone and in combination with hip-strengthening exercises (DNSHS) on balance, lower limb strength, and functional disability. Methods: In this randomized clinical trial, 32 older women with chronic low back pain were randomly assigned to two groups (n = 16 per group): DNS or DNSHS. Both groups participated in an eight-week intervention program. Balance was assessed using the Timed Up and Go (TUG) test, lower limb strength using the 30-Second Chair Stand test, and functional disability using the Roland-Morris Disability Questionnaire (RMDQ). Outcomes were measured before and after the intervention. Results: A significant time-by-group interaction was observed for balance (P = 0.039), lower limb strength (P = 0.001), and functional disability (P = 0.017), indicating different improvement patterns between groups. Bonferroni post-hoc analyses showed significant improvements within both groups from pre- to post-intervention: balance (P < 0.001) and lower limb strength (P < 0.001) increased, while disability decreased (P < 0.001). Conclusions: Both DNS and the combination of DNS with DNSHS improved physical function in older women with chronic low back pain. The distinct improvement patterns suggest that including DNSHS may provide additional benefits and could be recommended as part of rehabilitation programs for this population.en
