AbstractBackground: Despite achieving zero indigenous malaria cases in 2018 and 2019, Iran’s progress toward the 2025 malaria elimination goal has been hindered by several challenges. One of the key obstacles is the role of imported cases, particularly from migrants arriving from neighboring high-risk countries, such as Afghanistan and Pakistan. Objectives: This policy brief aims to highlight the significant barriers faced by migrants in accessing malaria prevention and to emphasize the importance of addressing these barriers in order to maintain Iran’s malaria-free status and achieve the 2025 elimination goal. Methods: This brief is based on published data regarding malaria cases and the challenges faced by migrant populations in Iran, with a focus on access to malaria prevention, healthcare resources, and living conditions. Results: The data highlight significant barriers migrants face in accessing malaria prevention, including a lack of awareness, inadequate healthcare resources, and living conditions that facilitate disease transmission. These factors contribute to the risk of malaria importation and threaten Iran’s malaria elimination progress. Conclusions: Addressing these gaps through comprehensive, targeted, and sustained policy interventions is crucial for maintaining Iran’s malaria-free status and preventing the reemergence of the disease.en
