Background: Problem-based learning (PBL) is an active educational strategy established on two bases, learner and problem. This study aimed to determine the effect of the Emergency Severity Index (ESI) triage education via PBL on the triage nurses' performance and the patients’ length of stay in the Emergency Department. Methods: In this interventional, the ESI-triage education was done via PBL for all 12 triage-nurses in the Besat Hospital. The tools were the observational time-recording checklist developed according to the Swailes et al. study and the ESI form. The validity of the tools was confirmed by experts. The observer reliability was defined by the inter-rater reliability. The Length of stay and ESI triage level of 72 patients before and 72 patients two weeks after intervention were recorded. Data was analyzed by t-test, MannWhitney and ANOVA with 95% confidence by the software SPSS-20.Results: The majorities of the triage nurses were men (75%), with a bachelor’s degree (91.7%), mean age 31.5±5.51 and work experience 6.72±8.00 years. The ESI-triage education via PBL strategy significantly lead to the decrease in the "triage by the nurses” duration (P

Prevalence of Extended-Spectrum Beta-lactamases (ESBLs) in Pseudomonas aeruginosa Isolated from Hospitalized Patients in Imam Reza and Imam Khomeini hospitals in Kermanshah, 2013‎
    (Brieflands, 2015-12-21) Fardin Zoghi Maleki‎; Mohammad Reza Nahaei‎; Nasrollah Sohrabi‎; Laya Rezavand‎
Background: Pseudomonas aeruginosa is one of the most common Gram negative bacteria involved in nosocomial infections. Presence of Extended-spectrum Beta-lactamases (ESBLs) genes, play an important role in induction of resistance in beta-lactamase  producing species to beta lactam antibiotics. Frequency of resistance to these antibiotics is increasing. In this study, antibiotic sensitivity and frequency of ESBLs in Pseudomonas aeruginosa which had been isolated from clinical specimens, was assessed by phenotypic methods. Methods: This study was performed on 200 isolated Pseudomonas aeruginosa from clinical specimens in Imam Khomeini and Imam Reza hospitals, Kermanshah during 2013. Specimens were determined and cultured by standard conventional methods. Antibiotic sensitivity test on 14 antibacterial agents was performed according to CLSI criteria. Then, frequency of ESBLs producing isolates was determined by Combined Disk Test (CDT) and Double Disk Synergy Test (DDST) methods. Results: This study revealed that the highest and lowest resistance rate were against to cefpodoxime (100%) and piperacilin-tazobactam (34%), respectively. Using CDT and DDST methods, 41% of isolates were ESBL positive. Conclusion: Considering high resistance rate of Pseudomonas aeruginosa to majority of antibiotics and increasing frequency of ESBLs producing isolates, we need for infection control criteria and use of appropriate therapeutic protocols according to antibiogram test patterns.

Methods: This is a cross-sectional descriptive study that is performed on nurses working in the private hospitals in Rasht, Iran. The data were collected through two questionnaires of organizational climate and job performance that its validity and reliability were confirmed. SPSS24 and descriptive and inferential statistics were used for data analysis. Results: According to the mean score obtained more than half of the nurses report a positive assessment of the climate in the hospitals. Also, less than one-third of nurses had a ideal level of performance regarding nurses' job performance. Also, the relationship between perceived organizational climate and the level of nurses' performance with the personal and occupational skills of nurses was emphasized. Finally, the relationship between perceived organizational climate and nurses' job performance was statistically significant (P ≤ 0.048). Conclusions: According to the results, the perceived organizational climate has a positive relationship with nurses' performance level. Therefore, managers and practitioners of private hospitals in Rasht should ultimately improve nurses’ performance by improving the organizational climate and by emphasizing variables such as providing work-related facilities, paying attention to nurses' interests, emphasizing team building, participating in decision-making, improving customer service, paying attention to hospital reputation improving relationships among staff and improving compensation practices.

The Effect of Resistance Training with Moderate and Progressive Intensity on Indices of Oxidative Stress (Serum 8-Isoprostane, Malondialdehyde and Reduced Glutathione Levels) in Young Healthy Men
(Brieflands, 2018-06-30) Ali Zabet; Farshad Ghazalian; Hojjatolah Nik-Bakht
Background: 8-isoprostane(8-IsoP) and malondialdehyde (MDA) are novel and classic index of lipid per oxidation, respectively, and reduced glutathione (GSH) is an index of antioxidant status, which the chronic adaptation of moderate and progressive resistance training on them is unclear. The purpose of this study is to investigate and compare the effect of 9 weeks moderate and progressive resistance training on serum 8-isoprostane, malondialdehyde and reduced glutathione levels in young men (from 17- to 21 year-olds). Methods: In the semi-experimental study, 36 volunteers were randomly divided in to 3 groups of 12 men involving: moderate and progressive resistance training and control groups. The training protocol involved three periods of three weeks each with progressive intensity. The moderate training group performed the first period of protocol in throughout of study (just 5 percent increase in training intensity as overload), while the training load of progressive training group was incrementally increased in every period. Blood sampling were taken in two phases involving pre test and following ninth week from anterior brachial vein. The variables were measured via sandwich ELISA. Results: The results of ANCOVA and Bonferroni tests showed that 9 weeks of moderate and progressive resistance training led to significance decrease in lipid oxidation indices (8-IsoP and MDA);the range of the decline especially in serum 8-IsoP was more in progressive group. Significant change in serum GSH levels was not observed in the training groups. Conclusions: The results of this study show that periodic increase in resistance training load leads to improves in oxidative stress adaptability. Moreover, 8-IsoP is more accurate index for predicting oxidative stress compared to MDA.

Comparing 10-Item and 60-Item Big Five Personality Inventories in Explaining the Depressive Symptoms in Patients with Acute Myocardial Infarction
(Brieflands, 2019-12-31) Mozhgan Saeidi; Keivan Kakabraee; Khodamorad Momeni‎
Background: Depression is one of the most probable side effects of myocardial infarction (MI) besides its sudden occurrence, and is affected by various factors, such as personality traits. Objectives: Hence, the present study was conducted to compare 10-item and 60-item big five inventories in explaining the symptoms of depression in patients with acute MI. Methods: The study population in this cross-sectional study comprised 250 patients with MI in Iran. The patients responded to 60-item NEO Five-Factor Inventory (NEO-FFI), 10-item Big Five Inventory- Short Version (BFI-SV), and depression subscale from Depression Anxiety Stress Scales (DASS) from April to September 2018. Data was analyzed using chi-square test, Pearson correlation coefficient, and multiple linear regression analysis. Results: Patients’ mean age (69.2% males) was 62.1 ± 11.1 years. The results showed a significant relationship between depression and factors of extraversion, agreeableness, and conscientiousness in BFI-SV and factors of extroversion, neuroticism, and agreeableness in NEO-FFI (P < 0.05). The results of regression analysis showed that BFI-SV explained 6.7% and NEO-FFI explained 5.8% of the variance of depression score (P < 0.05). Conclusions: BFI-SV explains the symptoms of depression in patients with MI as much as NEO-FFI. Thus, the researchers can use the 10-item version to assess the personality traits of patients with MI in future studies.
