Adaptation and Psychometric Properties of the Test for Instagram Addiction (TIA) Among Nursing and Midwifery Students in Clinical Learning Environments

AbstractBackground:  In recent years, excessive use of Instagram among students, particularly those in nursing and midwifery, has increased and is associated with compulsive behaviors, reduced self-control, and negative consequences. This can disrupt their concentration, clinical decision-making, and professional interactions. Objectives: This study aims to culturally adapt and validate the Test for Instagram Addiction (TIA) for use among nursing and midwifery students in clinical learning environments in Iran. Methods: A cross-sectional study was conducted in 2025 among nursing and midwifery students in clinical training hospitals in Kerman, Iran. The TIA was translated via a two-phase approach involving bilingual translators and experts in psychometrics, medical sciences, and linguistics. The instrument underwent content validity testing through expert evaluations and construct validation via exploratory and confirmatory factor analysis (EFA and CFA). Reliability was assessed via internal consistency (Cronbach’s alpha) and test–retest methods. Concurrent validity was examined by correlating TIA scores with scores from the General Phubbing Scale (GPS). The sample consisted of 400 students for EFA, 300 for CFA, and additional samples for reliability assessments. Results: The TIA demonstrated high content validity, with content validity ratios (CVRs) between 0.80 and 1.00 and content validity indices (CVIs) ranging from 0.83 to 0.97, indicating clarity, relevance, and alignment with the intended construct. The exploratory factor analysis identified six factors consistent with the original scale, namely, lack of control, disengagement, escapism, health and interpersonal troubles, excessive use, and obsession, explaining 65.3% of the total variance. The CFA confirmed the model’s good fit to the data, with fit indices exceeding the recommended thresholds. Reliability tests indicated excellent internal consistency (α = 0.919) and good test–retest reliability (r = 0.89, P < 0.01). Concurrent validity was confirmed through a strong positive correlation with the GPS score (r = 0.76, P < 0.01). Conclusions: The Persian TIA is a valid and reliable tool for assessing Instagram addiction in nursing and midwifery students in clinical settings and is recommended for use in research and clinical interventions.en
