Assessing the Diagnostic and Prognostic Value of RDW and MPV in Cerebral Venous Sinus Thrombosis: Insights from a Case-Control Study

AbstractBackground: Cerebral venous sinus thrombosis (CVST) is an uncommon cause of stroke, characterized by a diverse spectrum of clinical manifestations. Timely diagnosis and treatment are crucial for improving patient outcomes. Headache is the most frequent clinical manifestation of CVST, often necessitating diagnosis through high clinical suspicion followed by confirmation via appropriate imaging techniques. Objectives: However, given the constraints of imaging resources within the healthcare system, this study aims to explore more cost-effective diagnostic approaches by comparing the red cell distribution width (RDW) and mean platelet volume (MPV) between patients with confirmed CVST and a control group. Methods: This case-control study analyzed 41 patients definitively diagnosed with CVST and 55 controls. The study protocol was approved by the Ethics Committee of Zahedan University of Medical Sciences under the code IR.ZAUMS.REC.1401.223. It assessed RDW and MPV, using clinical and MRI data, with statistical tests conducted in SPSS to identify significant correlations. Data were collected using information from the patients' medical records, including MRI reports, demographic data, and patient laboratory results. Statistical analysis was performed using SPSS version 24, with significance set at P < 0.05. Results: 85.4% of participants in the CVST group had headaches. The mean RDW in the CVST group (45.58 ± 5.94) did not significantly differ from the control group (43.84 ± 3.61; P = 0.136). Similarly, MPV in the CVST group (9.51 ± 1.21) showed no significant difference compared to controls (9.58 ± 0.91; P = 0.76), indicating no statistically meaningful association between these parameters and CVST. Conclusions: The RDW and MPV did not differ significantly between the CVST and control groups. It is recommended that MR venography/CT venography (MRV-CTV) be employed as the diagnostic gold standard in CVST.en
